YEARBOOK PHOTO SUBMISSION FORM



All images submitted for retouching will be done on a first come first served basis. I like to have these done before Thanksgiving so that I can spend time with family.  Therefore, I respectfully ask for all submissions to be finalized and turned into me using the form below by November 11th.


Parents and their senior are responsible for making sure the yearbook image is submitted to the yearbook staff at your school. 


Please ask for a confirmation email from your school's yearbook coordinator showing that your senior yearbook photo has been received and that there is nothing more that the yearbook staff needs from you.
